What is Healthier Heart at GMHBA?
If you’ve recently been discharged from hospital after a cardiac episode, Healthier Heart at GMHBA is here to support your recovery. We have partnered with Cardihab to offer eligible* members with Silver Tier hospital cover (or higher), access to a 6-week evidence based digital cardiac rehabilitation program tailored specifically to your needs. Looking after your heart is important, and after a cardiac episode, having expert support and proven results, Healthier Heart at GMHBA is a step towards a healthier heart.
The program can help you regain strength, improve heart health and reduce the risks of future complications. It's designed to fit into your life, from the comfort of home.
*To be eligible for GMHBA to fund your participation in the Healthier Heart at GMHBA program you must be 18 years or older, hold an active Silver Tier Hospital policy (or higher), have served all relevant waiting periods, and have had a cardiac related admission in the last six months.

What's included in the program?
- Telehealth appointments with GMHBA’s cardiac nurses, combined with app support, to reduce the need for in-person visits.
- Guidance to help you safely increase physical activity as part of your recovery.
- Tips and support to build healthy eating habits and better understand nutrition.
- Stay on top of your medications with app-based reminders and alerts.
- Learn practical techniques to manage stress and reduce anxiety.
- Encouragement and support to develop long-term healthy habits, including quitting smoking and cutting back on alcohol.
Take control of your heart health
Your care will be supported by GMHBA’s health professionals and follows expert guidance from the Australian Heart Foundation and Australian Cardiovascular Health and Rehabilitation Association. The program works alongside your GP and specialists to support your ongoing recovery.
Your rehabilitation plan is tailored to your individual condition, recovery needs, and lifestyle. It includes guided exercises, education, and behaviour change strategies designed just for you.
The program promotes lasting lifestyle changes – like improved fitness, diet, and stress management – helping reduce the risk of future cardiac events and hospital readmissions.
Healthier Heart at GMHBA’s program is delivered via a secure mobile app, allowing you to complete your cardiac rehabilitation anytime, anywhere. No need for in-person appointments or travel. Eligible* members can access the app for 12-months post completion of the program.

Yes - the program is delivered via a secure mobile app, so you can complete your rehab from home via a compatible mobile, tablet or desktop.
You’ll need to have had a recent cardiac-related hospital admission (within the last 6 months) and meet the eligibility criteria (see above). Your healthcare provider can refer you, or you can use the contact form above to check eligibility. We'll work with your GP or specialist where needed.
To use the program, you’ll need internet access. While it works best on a smartphone or tablet, you can also access it using a desktop computer if needed.
For the best experience, we recommend using a smartphone or tablet running Android 9 or later.
Cardihab regularly updates its app to ensure it stays compatible with the latest Apple (iOS) and Google (Android) operating systems.
The Cardihab app meets international standards for keeping your personal and health information safe. It’s certified under ISO/IEC 27001:2013, which means Cardihab follow strict guidelines to protect your privacy and ensure high-quality, secure systems.
Once eligible* members finish the 6-week program, they’ll still be able to access the app for another 12 months.
