Joining GMHBA involves completing a simple application form. You can do this in a number of ways:

• Join online via our online application form. You'll need your Medicare card, details of your old health fund if you're switching, and your partner's and kids' information if you're signing up for couples or family cover.
• Call 1300 4 GMHBA (46422) Monday to Friday between 8am and 6pm (AEST). One of our friendly staff will help you with your application.
• Download a PDF version of the application form, print it out and mail it back to us at:
