Joining GMHBA involves completing a simple application form. You can do this in a number of ways:
- Join online via our online application form. You'll need your Medicare card, details of your old health fund if you're switching, and your partner's and kids' information if you're signing up for couples or family cover.
Start your application
- Phone us on 1300 4 GMHBA (46422) Monday to Friday between 8am and 6pm (AEST). One of our friendly staff will help you with your application.
- Drop into one of our branches and apply over the counter or complete the application form below and hand it to one of our helpful staff.
